Sağlık Hizmetlerinde Yönetişim İnovasyonu Sadece Bir “Moda” Kavram Mıdır?

Abstract

Amaç: Günümüzde sağlık kurumlarında sunulan hizmetin doğası gereği ve odağında insanın yer alması nedeniyle bu alan diğer endüstri ve hizmet sektörlerinden ayrılmakta; bu nedenle ‘sağlıkta yönetişim inovasyonuna’ özel bir önem atfedilmektedir. Konunun bu öneminden hareketle hazırlanan araştırmanın amacı; “sağlık hizmetlerinde yönetişim inovasyonunun bir ‘moda kavram’ olup olmadığının” araştırılmasıdır. Yöntem: Araştırmada “PRISMA Şemasından” yararlanılarak. “Sistematik Literatür Taraması” yöntemi kullanılmıştır. Bu yöntem aracılığıyla “sağlık hizmetlerinde yönetişim inovasyonu bir ‘moda’ kavram mıdır?” şeklindeki araştırma sorusuna yanıt bulabilmek için ulaşılabilen çok sayıda çalışmadan yararlanılmıştır. Bu aşamada toplanan bulgular, eleştirel olarak değerlendirilmiş, birleştirilmiş ve sistematik bir şekilde sunulmaya çalışılmıştır. Bulgular: Bulguların değerlendirilmesiyle ulaşılan sonuçlardan önemli birisi; “sağlık hizmetlerinde yönetişim inovasyonunun bir ‘moda kavram’ olmadığıdır”. Diğer önemli bir bulgu; “literatürde inovasyon, yönetişim ve kurumsal yönetişim olgularının giderek artan sayıda yayınla incelendiğidir”. Son ve önemli bir bulgu ise, “sağlıkta yönetişim inovasyonu konusunda çalışmaların çok sınırlı sayıda” olduğudur. Sonuç: Küreselleşme süreciyle birlikte yaşanan paradigma değişimi “yönetim” kavramından farklı bir yapıya sahip “yönetişim” olgusunu yeni bir model olarak sunmaktadır. Bu nedenle sağlık hizmetinin sunumunda “iyi bir yönetişim sisteminin tesis edilmesi” gerekliliğinin altı çizilmektedir. Araştırma kapsamındaki literatür sağlık kuruluşlarının sürdürülebilir yenilikleri benimseyerek maliyetleri düşürebileceğini, hasta sonuçlarını iyileştirebileceğini ve dolaysıyla sağlık hizmeti sunumunun etkinliğini ve verimliliğini artırabileceğini göstermektedir.Aim: Due to the nature of the service provided in healthcare institutions today and the fact that people are at the center of it, this field is distinguished from other industrial and service sectors; therefore, special importance is given to ‘governance innovation in healthcare’. The purpose of the research prepared based on this importance of the subject is to investigate “whether governance innovation in healthcare services is a ‘fashion concept’”. Method: The “PRISMA Scheme” was used in the research and the “Systematic Literature Review” method was used. Through this method, a large number of accessible studies were used to find an answer to the research question “Is governance innovation a ‘fashionable’ concept in health services?” The findings collected at this stage were evaluated critically, combined and presented in a systematic way. Findings: One of the important results reached by evaluating the findings is that “governance innovation in health services is not a ‘fashion concept’”. Another important finding is that “innovation, governance and corporate governance phenomena are examined in literature with an increasing number of publications”. A final and important finding is that “studies on governance innovation in health are very limited”. Conclusion: The paradigm shift experienced with the globalisation process presents the phenomenon of ‘governance’, which has a different structure from the concept of ‘management’, as a new model. For this reason, the necessity of ‘establishing a good governance system’ in the provision of health services is underlined. The literature within the scope of the research shows that by adopting sustainable innovations, healthcare organisations can reduce costs, improve patient outcomes and thus increase the effectiveness and efficiency of healthcare service delivery.
