Normal İşiten ve İşitme Kaybına Sahip Yetişkin Bireylerin Beck Depresyon Envanterine Göre Değerlendirilmesi ve Karşılaştırılması

Özet
Amaç: Bu çalışma, Beck Depresyon Envanteri ile değerlendirilen normal işiten ve işitme kaybına sahip
bireylerin depresyon seviyelerini karşılaştırmak, işitme kaybına sahip bireylerin ise işitme kaybının
unilateral/bilateral olmasına ve işitme kaybı derecesine göre depresyon seviyeleri arasında fark olup
olmadığını araştırmak amacı ile yapılmıştır.
Yöntem: 18-88 yaş aralığında normal işiten bireyler (n=40) ve en az orta derecede sensörinöral tip işitme
kaybına sahip bireyler (n=40) ile çalışılmıştır. İlk adım olarak İstanbul Gelişim Üniversitesi Etik
Kurul’undan izin alınmıştır. Çalışmanın power analizi yapılmış, gerekli örnek hacim 70 kişi olarak
belirlenmiştir (Her grup için 35 kişi). Gönüllü onam formu imzalatılan katılımcılara öncelikle saf ses
odyometri testi ve Speech Reception Threshold (SRT), SDS Speech Discrimination Score (SDS), Most
Comfortable Level (MCL) ve Uncomfortable Loudness Level (UCL) olmak üzere konuşma testleri
uygulanmıştır. Sonrasında ise yüz yüze Beck Depresyon Envanteri uygulanmıştır. Ölçekten alınan toplam
puan araştırmacı tarafından puanlanmıştır. Çalışmada toplanan verilerin analizi SPSS 25 programı ile
gerçekleştirilmiştir.
Bulgular: Çalışmaya katılan 40 işitme kayıplı birey çalışma grubu olarak alınırken 40 normal işiten birey
ise sağlıklı birey grubu olarak adlandırılmıştır. Çalışmaya katılan bireylerde ölçekten aldıkları ortalama
puana göre çalışma grubu ve sağlıklı birey grubu arasında istatistiksel olarak anlamlı farklılık
saptanmamıştır (p>0,05). Fakat çalışmaya katılan çalışma grubundaki bireylerde depresyon seviyesi
bakımından işitme kaybı unilateral olanlar ve bilateral olanlar arasında istatistiksel olarak anlamlı farklılık
bulunmuştur (p<0,05). Çalışmaya katılan çalışma grubundaki bireylerde depresyon seviyesine bakımından
işitme kaybı orta seviye olanlar ve ileri seviye olanlar arasında istatistiksel olarak anlamlı farklılık
bulunmuştur (p<0,05). Sonuç: İşitme kaybı gibi bir engele sahip olmak bireysel hayatta ve sosyal alanda uyumsuzluklara neden
olarak bireyin psikolojik durumunu etkileyebilmektedir. Araştırmamızda da özellikle işitme kaybının
bilateral olması ve işitme kaybı derecesinin fazlalığı ile depresyon seviyesinin de arttığı ortaya konmuştur.
Bununla birlikte yaş ile birlikte de depresyon seviyesinde bir artış gözlenmiştir. Yaş ile birlikte ve/veya yaştan
bağımsız olarak günlük hayatı etkileyecek problemlerin ortaya çıkması ruhsal yaralanmalara sebep
olmaktadır.
Aim: The aim of our study is to compare the depression levels of individuals with normal hearing and hearing loss evaluated with the Beck Depression Inventory and to investigate whether there is a difference between the depression levels of individuals with hearing loss according to the side of hearing loss and the degree of hearing loss. Method: Individuals with normal hearing (n=40) and individuals with at least moderate sensorineural hearing loss (n=40) between the ages of 18-88 were included in the investigation. As a first step, permission was obtained from the Istanbul Gelişim University Ethics Committee. Power analysis of the study was made, and the required sample volume was determined as 70 people (35 people for each group). The participants, who signed a voluntary consent form, were first administered a pure tone audiometry test and speech tests including SRT (Speech Reception Threshold), SDS (Speech Discrimination Score), MCL (Most Comfortable Level), and UCL (Uncomfortable Loudness Level). Afterwards, the Beck Depression Inventory was applied face-to-face. The total score obtained from the scale was scored by the researcher. The analysis of the data collected in the study was carried out with the SPSS 25 program. Results: While 40 individuals with hearing loss participating in the study were taken as the study group, 40 individuals with normal hearing were named as healthy individuals. There was no statistically significant difference between the study group and the healthy individual group according to the average score they got from the Beck Depression Inventory (p>0.05). However, a statistically significant difference was found between those with unilateral and bilateral hearing loss in terms of depression level in the individuals in the study group participating in the study (p<0.05). A statistically significant difference was found between the individuals in the study group participating in the study in terms of the level of depression between those with moderate hearing loss and those with advanced levels (p<0.05). Conclusion: Having a disability such as hearing loss can affect the psychological state of the individual by causing incompatibilities in individual life and social life. In our study, it was revealed that the level of depression also increased with the excess of hearing loss and the degree of hearing loss. However, an increase in the level of depression was observed with age. The emergence of problems that affect daily life with and/or independent of age causes mental injuries.
